South County Renewal Living a Recuperative Care Residence

Recuperative Care  -  Supportive Transitional housing  -  sustainable independent living

Recover. Rebuild. Renew.

A beautiful home, right here in our community. South County Renewal Living is a welcoming residence on a residential street in Gilroy where adults who are homeless or at risk can heal after an illness, injury or hospital stay. You are not a patient in a facility here. You are a neighbor who is recovering, surrounded by people who know your name and are here to help you get back on your feet. Our CHW's are here to help you navigate your benefits and get you the support you need to maintain your medi-cal benefits.

A Continuum of Integrated Care that connects two levels of support

Recuperative Care
Establishing stability. A safe, homelike place to heal from an illness, injury or a flare of a chronic or behavioral health condition. Residents receive onsite care coordination and case management supervised by our staff site nurse, a wellness check daily, 24-hour awake staff, three meals a day, and onsite access to nursing, wound care and medication management through our partner medical and home health providers. The steps toward renewal, healthing and path forward start on day one, not at discharge.

Supportive Transitional Housing
The step-down happens in the same residence, so no one has to move to keep making progress. Residents work on a housing plan, benefits and documents, employment and community connections, and Life Supportive Services workshops in budgeting, cooking, cleaning, hygiene and independent living.

Who We Plan to Serve

Target population

Adults experiencing or at risk of homelessness who need a safe place to recover from an injury or illness, including recovery from a flare of a chronic condition. This may include adults with behavioral health or substance use needs.

Individual adults

The home serves individual adults; we cannot accommodate children or families. Recuperative care is not designed for people who need help with daily activities such as bathing, dressing or eating, or who need skilled nursing or residential treatment; we will help those referrals find the right setting. Final eligibility criteria will be published before the home accepts referrals.

Our Approach

Dignity, rest and connection. We follow Housing First principles: admission is never conditioned on sobriety or treatment participation. Residents using medications for addiction treatment are welcome, with safe private storage. Our practice is trauma-informed, person-centered and culturally responsive.

Important Notes for Referral Partners

Please do not send patient or protected health information through this website.

Referral details and health information will be exchanged only through the secure referral process established and shared with approved partners before the program begins accepting referrals.

Participation as a referral partner does not begin until CRLF and the organization have established the appropriate referral process and any necessary agreements.

Listing an organization as a partner on CRLF's website requires that organization's permission.

Is bilingual support available?

Yes. Every resident has a bilingual Community Health Worker, and resident materials are in English and Spanish.

What about Housing First?

Admission is never conditioned on sobriety or treatment participation. Residents using medications for addiction treatment are welcome.

Where is the residence?

South County Renewal Living is being developed as a small, home-like residence located in Gilroy, California. The street address will remain private.

Can families be served?

At launch, the program will serve individual adults and will not be able to accommodate children or families.

Frequently Asked Questions

Is the home open now?

Not yet. The South County Renewal Living recuperative care home is coming Spring 2027. However,

Our Community Health Worker Services are available now.

Can my organization refer someone now?

To our CHW Services, yes. To the South Country Renewal Living , you can contact us now

about future referrals.

What will the home feel like?

Like home. It is a house on a residential street, with a shared kitchen, a living room and real bedrooms, not a clinic or a shelter. Residents live as neighbors in their own community, with a caring team on site day and night.

Does the home provide nursing or medical care?

The home is not a hospital or skilled nursing facility. Our site nurse is on site every day to supervise each resident's care plan, and residents have onsite access to nursing, wound care and medication management through our partner medical and home health providers. Residents keep their own doctors and specialists.

Is this permanent housing?

No. Both levels are short-term, and our team helps each resident work toward a stable home.

How long can someone stay?

Stays are based on each person's recovery and transition needs.

Stays are based on each person's recovery and housing needs and the limits of the funding source. Under Medi-Cal, recuperative care is limited to six months in any rolling 12-month period.

At launch, we will serve individual adults only and are not able to accommodate children or families.

What if someone loses their health plan or moves to fee-for-service Medi-Cal?

Their CHW helps them keep or restore coverage and works with us to find the right funding for their stay.

Can families or children stay?

At launch, the home serves individual adults only and cannot accommodate children or families.

Are services available in Spanish?

Yes. Every resident has a bilingual Community Health Worker, and resident materials are in English and Spanish.

Can I refer myself or a family member?

You can contact us about future referrals. Final referral criteria will be published before the home accepts referrals.

Is there a cost to residents?

We will work with partners and residence to determin the best funding source.
