Programs Designed Around the Journey Forward

CRLF runs two programs. Each stands on its own, and they work together when a person needs both.

Community Health Worker Services are available now, to any South County adult on Medi-Cal, whether they are in a health plan such as Santa Clara Family Health Plan or in fee-for-service Medi-Cal. No bed is required. The South County Renewal Living recuperative care home is a separate site, a homelike house in Gilroy, coming soon in 2027.

COMING SOON 2027

South County Renewal Living recuperative care home

Recuperative Care • Transitional Housing • A Real Home in Our Community

A small house on a residential street in Gilroy, furnished and run like a home. Adults who are homeless or at risk recover from an illness, injury or hospital stay at a kitchen table and in a bedroom of their own, close to their doctors, family and community, so they never feel sent away or set apart. When they are ready, they step down into transitional housing in the same bed.

Recover. Rebuild. Renew.

Growing With Purpose

CRLF's broader vision is to build connected, community-based programs that respond to gaps in health, recovery, housing stability, and other periods of transition. New programs and locations will be developed thoughtfully in response to community needs, partnerships, available resources,

and organizational capacity.

Level 1 — Recuperative Care

Safe recovery close to home

A safe, homelike place to heal for adults who no longer need a hospital but still need time to recover. Residents receive onsite care coordination and case management supervised by our site nurse, wellness checks every day, 24-hour awake staff, three meals a day, and onsite access to nursing, wound care and medication management through partner medical and home health providers.

Level 2 — Supportive Transitional Housing

The same bed, a planned next home

The step-down happens in the same bed, so no one has to move to keep making progress. Residents work on a housing plan, benefits and documents, employment and community connections, and Life Supportive Services workshops in budgeting, cooking, cleaning, hygiene and independent living.

Adult TAY Renewal Pathway

Developmentally responsive support for adults ages 18 to 25

A future pathway for adults ages 18 to 25 navigating homelessness, foster care transitions, behavioral health challenges or justice involvement, coordinated with county child welfare, extended foster care and THP-Plus rather than duplicating them.

Expanding Beyond

South Santa Clara County is where CRLF begins.

We will learn here, measure what works, and bring the model to other California communities where the same gaps exist. Each new pathway will open only after the first home meets its proof points. CRLF does not represent any service as licensed, contracted or reimbursable until it is.

A Collaborative Model

CRLF believes housing and health work best when the right people work together. Health plans, clinicians and county behavioral health bring the care and expertise. CRLF brings the dedicated Community Health Worker, the homelike setting and a community of peers.

Supportive Transitional Housing

The same residential setting, once a resident's focus turns to housing: a housing plan, benefits and document readiness, Life Supportive Services workshops in budgeting, cooking, cleaning, hygiene and independent living, and a planned move to a stable home.

One Home. Two Connected Levels of Support.
