A Safer Next Step After Discharge

South County Renewal Living is being developed to partner with hospitals, health plans, care management teams, county programs, and community organizations serving adults who are ready to leave acute care but do not yet have a safe place to recover.

Our goal is to help create a more coordinated transition from healthcare to recovery, housing navigation, and a stable next step—right here in South County.

What We Are Building Toward

  • Safe, respectful discharges for adults who no longer require acute medical care.
  • Fewer avoidable returns to the ED or hospital.
  • Consistent follow-up on recovery plans and primary care connections.
  • Continuity from recovery into housing navigation and stability.
  • Bilingual support and community-based coordination.
