We believe good programs do more than provide a bed to sleep in. They create an environment where people are treated with dignity, expectations are clear, relationships matter, and every person has the opportunity to move forward.
CRLF brings together thoughtful supportive housing, community partnerships, care coordination, and peer connection to support recovery, stability, and renewal.
Our role is not to replace the healthcare or behavioral health professionals serving our residents. We create the home and environment where residents can continue receiving the care they need while working toward a more stable next step.
Our Approach

Who We Are
California Renewal Living Foundation is a South County nonprofit built on a simple belief: no one should have to face a hard season alone. Our bilingual Community Health Workers and peer partners stand at each person's side, advocating for them and helping them through whatever life is putting in front of them right now, one conversation, one form and one appointment at a time.
Our work has two parts. Community Health Worker Services help members keep Medi-Cal, use it, and stay connected to their doctors, behavioral health care and community resources.
The South County Renewal Living recuperative care home gives adults who are homeless or at risk a homelike place in our own community to heal after an illness or hospital stay, and a steady step toward housing.
We are a bridge, not a replacement. Health plans, hospitals, county behavioral health, clinics and community organizations keep the work they are funded and equipped to do. We make sure the person in the middle does not fall through the gaps between them.

Why We Exist
The gaps show up when a renewal letter goes to an old address, when a member is moved to a new kind of Medi-Cal and does not know which doctors still take it, when someone leaves the hospital with nowhere safe to heal, or when a person living with serious behavioral health needs is trying to hold everything together alone.
In 2027 those gaps get wider. Many adults will renew Medi-Cal every six months and report work, school or volunteer hours to keep it. Members with unsatisfactory immigration status move to fee-for-service Medi-Cal and lose the care management their health plan provided. CRLF exists so the people with the least room for error have someone who knows the way, answers the phone, and stays with them until they are through.

Rooted in Our Community
Our unique South County landscape is rural, intimate, and rich in culture and heritage, yet often separated from essential services in San Jose. We prioritize listening and deep local collaboration to bridge this gap. Our recuperative residence is nestled within a quiet neighborhood, ensuring individuals heal with dignity as fellow neighbors rather than being sent to an unfamiliar location without the community and support they deserve.

Where We're Going
Community Health Worker Services are available now. The South County Renewal Living recuperative care home is coming spring 2027. We will learn here, measure what works, and grow carefully as partnerships and funding allow.
We envision a California where every community offers a dignified path from crisis to renewal, where recovery has a safe place to happen, housing is connected to health, and every person can reclaim confidence, belonging and a future worth building.
Bilingual by Design
Language should never be a barrier to feeling welcome, informed and respected. CRLF works in English and Spanish in the CHW program and in the home, arranges qualified interpretation in other languages, and uses plain-language materials. We serve members regardless of immigration status.
Nuestros servicios y materiales están disponibles en inglés y español, sin importar su estatus migratorio.
