Partnership Opportunities:
A Collaborative, Complementary Model

California Renewal Living Foundation (CRLF) partners with health plans, healthcare providers, and community organizations through a model that complements existing services rather than duplicates them. Our partners continue to provide the clinical care, health plan administration, and specialized services within their scope, while CRLF serves as a trusted community-based partner addressing gaps in access, coordination, and housing stability.
CRLF provides bilingual Community Health Worker (CHW) services to help members navigate Medi-Cal renewals, understand applicable work-reporting requirements, maintain coverage, and connect with appropriate healthcare and community resources.
South County Renewal Living, a community-based recuperative care program with a planned transitional housing step-down, designed to complement applicable 2027 Recuperative Care and housing-related Community Supports, including Transitional Rent.
Our goal is to help partners do what they already do more effectively by handling the community-based support, navigation, and housing coordination that members often need beyond the clinical setting. This collaborative approach supports continuity of care, reduces avoidable gaps in coverage and services, and creates a more coordinated pathway from healthcare recovery to long-term housing stability in South Santa Clara County.
Medi-Cal Fee-for-Service Partnerships
Don't Lose Your Medi-Cal Benefits
Beginning January 1, 2027, certain Medi-Cal members with UIS may transition to fee-for-service Medi-Cal, affecting access to managed care services like ECM and Community Supports.
CRLF's bilingual Community Health Workers (CHWs) are ready and available to help you navigate this transition, understand your coverage, connect with providers, and access available services.
Stay connected. Protect your coverage.

County and Behavioral Health Partnerships
County Behavioral Health Services, Full Service Partnership and substance use providers keep treatment and clinical risk management. CRLF offers CHW navigation and a housing pathway designed to fit BHSA Housing Interventions for people with significant behavioral health needs who are homeless or at risk. Reentry and probation partners keep supervision and the reentry case plan; CRLF provides coverage support, stabilization and a housing pathway.
Hospital and Community Partnerships
Hospitals and discharge planners keep the clinical discharge plan and medical oversight; CRLF provides a local, homelike place to recover, a warm handoff and outcome reporting.
Employers, colleges and community organizations bring specialized services; CRLF brings member access,
referrals and coordinated goals.
We welcome referrals today to our Community Health Worker Services, especially members facing a renewal, a work-reporting deadline or the move to fee-for-service.
Referrals to the recuperative care home open in 2027.
