Community Health Worker Services
Keep Your Coverage • Navigate Care • One Worker, Start to Finish

AVAILABLE NOW
Medi-Cal is changing in 2027, and you should not have to figure it out alone. Our bilingual Community Health Workers are here to guide you: we read the letters with you, explain what they mean, fill out the forms together, find doctors who accept your coverage, and get you to your appointments. One person who knows your name and your story, from your first call until you are steady.
Free, in English and Spanish, wherever you are.

What Is Changing in 2027
Six-month renewals: many adults on Medi-Cal will renew every six months instead of once a year.
Work and community engagement reporting: starting January 1, 2027, many adults ages 19 to 64 will need to show 80 hours a month of work, school, volunteering or a work program, or qualify for an exemption, to keep Medi-Cal.
Move to fee-for-service: on January 1, 2027, members with unsatisfactory immigration status move from their health plan to fee-for-service Medi-Cal. Their coverage continues, but they need doctors and pharmacies that accept fee-for-service, and the health plan's care management ends.
If any of this feels confusing or worrying, that is exactly why we are here. Your CHW will check what applies to you, mark every deadline, and walk through each step with you before anything is due.

Two Ways We Help
Coverage Help — short-term, focused support for one task: a renewal packet, reporting hours, an exemption, a lost Benefits Identification Card, or finding a fee-for-service doctor after January 1.
Full CHW Support — ongoing support from one dedicated CHW for members with complex needs: chronic conditions, behavioral health or substance use needs, a recent hospital stay, or housing instability. Your CHW stays with you from first contact to lasting stability, with a named backup who knows your plan.
